You are making breakfast and have two oranges on the counter. You could peel and eat them—or squeeze several oranges into a glass and drink the juice in a few minutes.
The fruit has not suddenly become “bad.” The vitamins have not disappeared. But the physical form of the food has changed, and that changes how you eat it, how much fibre reaches you, how quickly you can consume it and how filling it may feel.
Quick Clarity
Whole fruit and 100% fruit juice can both provide fruit-derived nutrients, but they are not nutritionally interchangeable. Whole fruit generally retains more of its intact fibre-rich structure, requires chewing and tends to be more filling. Juicing removes or substantially reduces much of that structure and fibre, making the fruit easier to consume rapidly. WHO also classifies sugars naturally present in fruit juice as free sugars, unlike sugars held within the intact cellular structure of whole fruit.
That is why the useful question is not:
“Is fruit juice unhealthy?”
It is:
“What changes when I turn fruit from something I eat into something I drink?”
Start With the Fruit Matrix
A whole orange, apple, mango or pear is more than a container of sugar, vitamins and water.
Its nutrients are organised inside a physical structure made from plant cells, cell walls, fibre and water. You normally have to bite, chew and physically break that structure apart before swallowing it.
That structure matters.
When fruit is juiced, much of the insoluble fibrous material may be separated from the liquid. Depending on the fruit, equipment and whether pulp is retained, some soluble fibre and suspended material can remain, but juice generally contains substantially less fibre than the corresponding whole fruit.
The liquid also requires little or no chewing.
Those changes affect the food matrix—the physical architecture in which the nutrients originally existed.

Juicing does not simply make fruit smaller. It changes the way the fruit is physically delivered to you.
That distinction explains much of what follows.
What Happens to Fibre?
Dietary fibre is one of the clearest differences between whole fruit and juice.
In whole fruit, fibre is distributed through the edible plant tissue. It contributes to the fruit's physical bulk and forms part of the structure surrounding sugars and other nutrients.
During conventional juicing, much of the pulp and fibrous tissue is removed.
So a glass of juice can still contain:
- water;
- naturally occurring fruit sugars;
- vitamins;
- minerals;
- organic acids;
- plant compounds;
while containing considerably less fibre than the whole fruit from which it came.
That is not the same as saying juice contains “nothing useful.”
It means that one important part of the original food has been reduced or removed.
This is also why simply adding a small amount of isolated fibre back into a juice does not necessarily recreate the physiological experience of eating the original fruit. Older and more recent controlled studies comparing fruit in different physical forms have found that intact fruit can be more satiating than purée or juice.
Structure Changes How Quickly You Can Eat the Fruit
Imagine eating three oranges.
You need to peel them, separate the segments, chew them and swallow them.
Now imagine drinking the juice extracted from those oranges.
The liquid can be consumed much more quickly.
This sounds obvious, but it is nutritionally important.
A classic experiment comparing apples in different physical forms found that fibre-free juice could be consumed much faster than intact apples.
Later controlled research has reached a similar broader conclusion: processing apples into purée or juice speeds gastric emptying compared with eating the whole apple, while the whole apple produces greater feelings of fullness and satiety.
So the difference is not merely:
fibre versus no fibre.
It can also involve:
chewing + eating rate + physical volume + gastric behaviour + satiety.
That is why food structure deserves its own place in the comparison.
Whole Fruit Usually Makes Fullness Work Harder for You
One of the most practical differences appears before we even discuss blood glucose.
It is how full the food makes you feel.
In a controlled meal study, eating whole apple before a meal increased satiety more than applesauce or apple juice. Interestingly, adding naturally occurring amounts of fibre back to the juice did not reproduce the same satiety response as eating the intact apple.
That is a useful finding because it reminds us not to reduce the entire difference to a single nutrient.
The whole fruit offers:
fibre + intact structure + chewing + slower consumption + physical bulk
as one eating experience.
Juice offers a different experience.
That does not mean nobody should drink juice.
It means a glass of juice should not automatically be mentally counted as equivalent to chewing the same fruit.
What About Blood Glucose?
This is where oversimplification becomes especially easy.
You may hear:
“Juice spikes blood sugar, while whole fruit does not.”
That wording is too absolute.
Whole fruit contains carbohydrate and can also raise blood glucose. Juice does not produce one universal response either. The response depends on the fruit, amount, processing method, meal context and the person consuming it.
But structure can influence the metabolic response.
Earlier controlled experiments comparing whole fruits with their juices reported differences in insulin and post-meal glucose patterns. More recent work continues to show that whole-fruit structure, fibre and physical form can affect digestion and satiety.
The safer conclusion is:
Do not use one glucose number to define the whole food—but do not assume that turning fruit into liquid leaves its physiological behaviour unchanged.
For people managing diabetes or another medical condition, individual juice intake and carbohydrate choices should follow appropriate clinical guidance rather than a general internet rule.
Why WHO Treats Juice Sugars Differently
There is another distinction that surprises many shoppers.
WHO defines free sugars to include sugars naturally present in fruit juices and fruit juice concentrates.
Sugars contained within the cellular structure of intact fruits and vegetables are not treated the same way under that definition. WHO recommends keeping free sugars below 10% of total energy intake, with a further reduction below 5% suggested for additional benefit.
Why does this matter?
Because “no added sugar” and “not a free sugar” are not the same statement.
A bottle of genuine 100% orange juice may contain no added sugar.
Yet its naturally occurring juice sugars still fall within the WHO free-sugar definition.
That does not turn 100% juice into a soft drink.
It simply prevents the phrase “naturally occurring” from becoming a free pass on quantity.
Juice Is Not the Same as a Sugar-Sweetened Fruit Drink
This distinction is essential.
100% fruit juice and a fruit drink with added sugar should not be treated as identical products.
A 100% juice is derived from fruit and can provide nutrients naturally present in that fruit.
A fruit drink may contain:
- water;
- some juice;
- sugar or syrup;
- flavourings;
- other ingredients,
depending on the formulation.
Evidence on long-term health outcomes also requires calibration. For example, one meta-analysis found that sugar-sweetened fruit juice was associated with higher type 2 diabetes risk, while 100% fruit juice was not significantly associated in that particular analysis.
Other large observational research has reported different associations for whole fruit and fruit juice, reinforcing that the two forms should not simply be treated as nutritionally identical.
The useful lesson is therefore not:
“All juice is bad.”
It is:
Read what the product actually is.
What About Smoothies and Blended Fruit?
Blending creates another category.
A blender can retain much more of the fruit's edible material than a juicer because the pulp is not necessarily removed.
So:
whole fruit ≠ blended fruit ≠ strained juice
They should not automatically be collapsed into one category.
But keeping the fibre does not mean nothing has changed.
Blending physically disrupts the fruit structure and can make a larger amount of fruit quicker to consume. Research comparing blended and whole fruits has produced findings that depend on the specific fruit and study conditions, so it would be too simple to say that every smoothie behaves metabolically like juice.
A useful rule is: Ask both what was retained and what was disrupted.

The most important row may be the one that is not explicitly about nutrients:
Portion awareness.
Liquid fruit makes it easy to consume the equivalent of several pieces of fruit without experiencing the same chewing or fullness.
The Portion Illusion
Picture a small glass of orange juice.
It does not look particularly large.
But the meaningful comparison is not:
one orange versus one glass
unless that glass genuinely contains the juice from only one orange.
The better question is:
How much fruit went into this serving?
This is one reason juice portions matter.
A liquid can compress several pieces of fruit into a form that takes less time and effort to consume.
The fruit has not become nutritionally meaningless.
The dose and delivery format have changed.

Before treating a fruit product as equivalent to whole fruit, ask four questions.
1. What form is it?
Whole fruit?
Blended whole fruit?
100% juice?
Juice from concentrate?
Fruit drink?
Nectar or another formulated beverage?
The category matters.
2. What happened to the pulp and fibre?
Was the whole edible fruit retained?
Was pulp removed?
Does the nutrition panel show meaningful dietary fibre?
The answer helps you understand how much of the original structure remains.
3. How much fruit am I consuming?
Do not judge only by the size of the glass.
Consider how much fruit contributed to it and how easily the serving can be consumed.
4. What role is this playing?
Is the juice:
- an occasional beverage;
- accompanying a meal;
- being used because whole fruit is unavailable;
- replacing water;
- or routinely replacing whole fruit?
The answer changes the decision.
What UAE Guidance Says
This whole-fruit preference is not merely an Eatwise position.
The UAE Ministry of Health and Prevention advises people to eat whole fruits instead of juice to benefit from fibre as part of its healthy-diet guidance.
That is a particularly useful message in a market where fresh juices, juice blends and fruit beverages are widely available.
It does not require banning juice.
It simply establishes the everyday hierarchy more clearly: whole fruit first when practical; juice understood as a different format rather than a perfect substitute.

The right-fit conclusion is simple:
The purpose of eating fruit is not just to obtain fruit sugar and vitamins. The structure in which those nutrients arrive is part of the food too.
